Also, we have included driving directions for those coming from a distance away.
Bring -- your insurance card, -- valid driver's license or legal photo ID, -- current insurance card, written consult request from the referring doctor, -- address and phone for referring doctor and your primary care doctor (if different), -- any medical records that you have that may be relevant to the reason for this consult.
