New Patient Intake Documents All new patients must complete Intake Documents. Patients are asked to enter their medically relevant histories using the new computer linked pens while in the waiting room prior to your appointment. Please arrive 15 to 30 minutes ahead of your scheduled appointment so that you may complete the intake information and we may have time to upload it for the doctor to review with you. The Patient Intake documents on this site are only to be used for Dr Hawkins-Holt and Dr Zan new patients, when instructed.
